With Real Madrid constantly linked with Chelsea star Eden Hazard, a report has emerged today that states the club will use one of two players in a swap deal in order to land the Belgian.

With the club's most important player in recent seasons reluctant to extend his stay at Stamford Bridge, former Blues favourite Wise has outlined the conditions that Chelsea must achieve in order to get Hazard's signature.

However, Chelsea will not let Hazard go easily.

The 27-year-old makes sense as a priority target for Real after they sold Cristiano Ronaldo in the summer, with players like Gareth Bale and Marco Asensio failing to step up in his absence.

Manager Maurizio Sarri has tended to opt for Ross Barkley or Mateo Kovacic alongside N'Golo Kante and Jorginho, meaning Loftus-Cheek has been confined to just four league appearances off the bench.

Chelsea are set to offer Hazard a huge new contract in a bid to keep him at Stamford Bridge.

Cavs trade Hill, Dekker for Dellavedova, Henson & two draft picks
Now that George Hill and Sam Dekker have reportedly been dealt to the Bucks, it looks like it may be JR Smith's turn to move. In 12 appearances this season, Dellavedova is averaging 1.7 points, 0.8 rebounds and 2.4 assists per game.

"I want him here, of course, but I want him here if he wants to stay here", Sarri said in a press conference.

"He's a fantastic player", the Belgian told Sky Sports News .

"The club is available to sign a new contract immediately but it's up to him if he wants to renew".

Hazard, whose contract with Chelsea expires in 2020, admitted earlier this year that it has always been a dream of his to play for Los Blancos.

West London side Chelsea suffered a 2-1 loss against Wolves in their last Premier League game and are again faced with a tough game against league leaders Manchester City.
